Topics of discussion: Advance Directives, Medical Power of Attorney and Out of Hospital DNR.

• The Facilitator offers assistance in completing paperwork as needed and general education regarding each document and the importance of preparation.
• ALL HTMG patients and their families are encouraged to attend.
• Classes are offered every month at different clinics rotating between Alamo Heights, Blanco, Wurzbach, Helotes, Holy Cross and Val Verde. These classes are conducted by Compassus Hospice & Palliative Care and Vitas Hospice Care.
• The class is approximately 1 hour long.
• The clinic manager is notified the month before the class is to be hosted at their location via email request.
